- A: Continued progress toward anxiety reduction goal; coping skills used effectively. No acute safety concerns.
- P: Continue cognitive restructuring; client will practice 10 minutes of daily breathing. Follow up in 2 weeks.
Adapting the template for telehealth
For virtual sessions, note the modality (telehealth) and any observation limitations. Your observations still count — affect, engagement, and reported symptoms translate to video — but be specific about what you could and could not assess.
