What is a DAP note?

Many therapists find DAP notes faster to write because there is less redundancy than SOAP, while still giving payers and supervisors the clinical information they need.
The three components
D — Data: Everything you gathered in the session: the client's reported symptoms and concerns, direct quotes, your observations of affect and behavior, and any measures or scores.
A — Assessment: Your clinical interpretation. How the client is progressing toward treatment goals, the effectiveness of interventions, changes in presentation, and any risk assessment.
P — Plan: Next steps — interventions to continue, skills to practice, homework, and scheduling. Document safety planning when relevant.
Example DAP note
- D: Client arrived on time, reported stress at work and difficulty asserting boundaries with a supervisor. Described increased irritability at home. Stated mood 5/10. No SI/HI. Affect congruent, speech pressured at times.
- A: Client demonstrates insight into the link between unassertive communication and resentment. Progressing toward goal of improving interpersonal effectiveness. Mild anxiety symptoms persist.
- P: Continue role-playing assertive communication. Client will practice one assertive statement daily. Review next session; monitor mood.
